Question: Why are there so many letters in LGBTQ-etc.?

Isn't it true that every human being living or ever born, including even hermaphrodites, is or was identified as either male or female by having either xx or xy chromosomes? Are there more sex-determinate chromosomes in the human gene? Has any other combination ever been observed in all of human history?
